Residency and Secondary Tuition

Here is where you will find the necessary forms to request tuition support for secondary school attendance.

Please fill out the Tuition Request form completely and return to the address below via mail or fax. 

If you are new to your town, please also fill out the Declaration of Residency form and return along with the tuition request form.

Completed tuition requests and the appropriate residency verification materials should be mailed or faxed to:

NOTICE OF MANAGEMENT PLAN AVAILABILITY

The Asbestos Hazard Emergency Response Act (40 CFR 763.93 [g][4]) requires that written notice be given that the following schools have Management Plans for the safe control and maintenance of asbestos-containing materials found in their buildings. These Management Plans are available and accessible to the public in our office and at the administrative office of each facility listed below.
